We work with primary care providers to help them understand and work with patients with preventive, early-intervention, mild, and moderate behavioral health needs. This includes screening, brief intervention, case management, and referral support when needed.
Our Behavioral Health Consultants (BHCs) utilize a team approach that consists of the PCP, a social worker/counselor, and a psychiatric provider to provide ongoing support and direction to patients to ensure that they are receiving the best level of care.

BHCs work with families to complete screens for ADHD, anxiety, depression, substance use, maternal health and depression, parent support, child behavior, and many other concerns
When appropriate, BHC staff will work with children and families with brief interventions in the primary care setting and online. Interventions may include our COPE program, Food Matters, Family CheckUp4Health, and other skill-building programs to support families
Whether we are working with the family in the primary care setting or helping them find a more appropriate level of care, the BHC works with families to help manage concerns and provide support in completing referral options. The BHC helps with referrals, family support, and Social Determinants of Health (SDOH) concerns
